Diabetic Macular Oedema

چکیده

Diabetic macular oedema (DMO) is a major cause of blindness that has an increasing prevalence associated with the general population in diabetes worldwide. In DMO, increased expression of factors including vascular endothelial growth factor (VEGF) disrupts the blood– retina barrier causing leakage and accumulation of extracellular fluid in the macula with resultant loss of vision. Focal and grid laser photocoagulation has been used to treat DMO for many years, but can cause scarring and atrophy. Vitrectomy is also associated with undesirable effects and its use is mainly confined to tractional DMO cases. Intravitreal steroids have been used successfully but are associated with increased intraocular pressure and cataracts. The intravitreal anti-VEGF treatments have shown improved vision and reduced macular thickness in DMO with good tolerability. Among these, ranibizumab is the only treatment approved in the US and EU for this indication. Its use is supported by extensive clinical trials in which it was shown to be effective with or without laser treatment. Ranibizumab is cost-effective and is likely to have a continuing role in DMO therapy.

Retinal vessel dilatation and elongation precedes diabetic macular oedema.

AIMS/BACKGROUND Retinal vessel dilatation is a well known phenomenon in diabetes. In this study, the theory of whether excessive changes in diameter and length of retinal vessels occur in the development of diabetic macular oedema was tested, supporting a hypothesis that the development of diabetic macular oedema may be linked to hydrostatic pressure changes described in Starling's law. METHO...

Impact of metabolic control on macular thickness in diabetic macular oedema

PURPOSE To investigate the impact of metabolic control on macular thickness measured using optical coherence tomography in patients with diabetic retinopathy with or without macular oedema. METHODS A total of 124 patients with diabetic retinopathy ( n = 70 without macular oedema and n = 54 with macular oedema) were enrolled. Optical coherence tomography parameters measured included central ma...

Intravitreal triamcinolone acetonide for the treatment of diffuse diabetic macular oedema--a case report.

INTRODUCTION Although laser photocoagulation is the primary treatment for diabetic macular oedema, treatment of eyes with diffuse macular oedema has been disappointing. Intravitreal injection of steroids is being investigated for the treatment of diabetic macular oedema. Preliminary results indicate that steroid injections do improve macular oedema, but it is not clear if they improve visual ac...

Intravitreal triamcinolone versus laser photocoagulation as a primary treatment for diabetic macular oedema - a comparative pilot study

BACKGROUND Diabetic macular oedema is the leading causes of blindness. Laser photocoagulation reduces the risk of visual loss. However recurrences are common and despite laser treatment, patients with diabetic macular oedema experienced progressive loss of vision. Stabilization of the blood retinal barrier introduces a rationale for intravitreal triamcinolone treatment in diabetic macular oedem...

Vitrectomy for cystoid macular oedema with attached posterior hyaloid membrane in patients with diabetes.

AIM To report the success of vitrectomy in eliminating cystoid macular oedema and improving vision in three eyes of two patients with diabetic cystoid macular oedema. In all of the eyes there was no ophthalmoscopic evidence of traction from a posterior hyaloid membrane or from proliferative tissue. METHODS Pars plana vitrectomy was performed on three eyes of two patients with diabetic cystoid...
